Parent Permission Form (Alternate Transportation)
This form must be completed and given to the coach prior to taking your child home from an away game.
Student athletes who are interested in trying out or joining a team will need to have all required paperwork turned in by the first day of practice/tryouts. Registration and payments can now be made online.
MUKWONAGO HIGH SCHOOL ATHLETICS:
Athlete Participation Cost:
- $100 per sport
- $300 max per individual
- $400 max per family
Athletic Participation Requirements:
Student athletes who are interested in trying out or joining a team will need to:
1.) Complete the NEW online athletic registration and make payment by the first practice
2.) Physicals must be valid and on file after April 1st of the current school year
3.) Pay required fees online or by check made out to MHS
4.) Review and complete the Random Drug Testing Consent Form once per high school career
